Responsibilities of a Hospice Nurse:
When it comes to facing the reality that all life will one day come to an end, it can be difficult for families to come to terms with their grief. That is why we provide bereavement support to our patients and their loved ones. We endeavor to help them overcome the pain of loss and learn how to handle emotional and psychological triggers.
We assigned patients and their families to skilled case managers to help them through hospice care coordination. We work closely with the patients and develop a specialized care plan to improve their quality of life and enable them to live their final days comfortably.
Patients who suffer from severe illness and disease often suffer the pain of their symptoms and are unable to live their lives comfortably. For patients who wish to end medical care and transition to end-of-life care, we help them manage their symptoms so that, as much as possible, we can ensure their final days are spent comfortably.
Our health professionals work closely to evaluate the patient’s condition, monitor symptoms, and, from there, develop a specialized care plan geared towards managing their pain and improving their quality of life.
